General

All Registration Deposits are NON-Refundable, and transfer of registration is Strictly Prohibited.
All sizes submitted are FINAL.

We are not responsible for errors on the application form, so Please review and ensure you have
correctly entered your information. No exchanges are permitted for incorrectly sized items.
Collection of costumes require a FULL payment receipt. Should you require another person to
collect your costume; a clear copy of your photo ID, a signed permission letter and your FULL
paid receipt will be needed for collection. NO EXCEPTIONS.

Please Note: Any deposits via the Installment Payment Plan option, are Non-Refundable.
Any deep discounted costumes are not entitled to full amenities (hot food, top liquor).

Personalizations

-All major personalization of Foreday costumes must have the COMPLETE theme logo/brand
visible. No exceptions. No elaborate upgrades or add-ons to Kadooment costumes without
prior authorization from bandleader and/or designer.

Note: The costumes on display at the band house and online, are Original prototypes, and are
subject to minor changes during the production process, due to material availability.
Masqueraders are responsible for any minor adjustments to their costumes. Minor adjustments
can be considered as: - Adjustments to sizing of the head piece, neck, chest, arm and leg pieces
or belt, as well as alterations to panty/bra/pants to ensure a comfortable fit.

Declarations

By using this website, I acknowledge the following:

I understand that I am responsible for the payment of my selected costumes as laid out in this registration form and I am fully understanding of the terms and conditions.
I HEREBY ASSUME ALL THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH Ensayne Entertainment, and all who are being represented, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities, being released from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
I understand that I am responsible for my actions, and any negligent or purposeful act to cause/incite harm to self or others, will result in my removal from the band, and absolve Ensayne Entertainment from any liability from my actions.
I certify that I am physically fit, and am freely willing to participate in this activity, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity.
I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.
I, registered patron, do hereby consent and agree that Ensayne Entertainment and the National Cultural Foundation, its employees or agents, have the right to take photographs, videotape, or digital recordings of me and to use these in any, and all media now or hereafter known. I release all rights to exhibit this work in print and electronic form publicly or privately and to market. I understand that there will be no financial or other remuneration for recording me.
I represent that I am at least 18 years of age. If under the age of 18 years old you, must bring a parent or guardian to sign a release form, before the costume is released. Please note: A consenting adult must accompany you for the parade.
Due to Covid-19, I understand Festival/Government rules may change without advance notice and Ensayne Entertainment will not be held responsible for personal changes affected by official policies.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT, AND I AM AGREEING OF MY OWN FREE WILL.
